STEP 1

Please read the TERMS AND CONDITIONS page below, then complete the registration form for medication education. By typing YES in the last box, you will confirm your acceptance of the TERMS AND CONDITIONS.

You will receive an email with several proposed dates and times for your medication education sessopm. Please choose a date/time and return the email along with a list of the client's current medications.

STEP 3

You will receive an email confirming the date and time for your medication education session. Once you submit the $20 deposit along with the registration form for medication education, the appointment time is confirmed.

STEP 4

STEP 3

After the medication education session, you will submit the $45 balance. Upon completion, you will receive the custom medication list and senior medication safety information sheets.
